Human in the loop means a person reviews, approves, or corrects what an AI system produces before or after it acts. It differs from handoff: the customer is not transferred to a human, the human sits behind the answer. The review point adds human oversight to an automated workflow.

  • After the answer. The agent replies first, while a person later reviews a sample of completed conversations for unsupported answers, missed policy changes, or model drift. The review can improve future behavior, but it cannot correct an answer before the customer receives it.
  • On exceptions only. The agent retrieves source material and handles requests that pass its rules and confidence threshold, while output below the threshold is escalated to an internal review queue. The reviewer can approve, correct, or reject the proposed answer or action without taking over the customer conversation.

Why Reviewing Every Answer Creates Another Queue

Most teams want exception-only review but buy pre-answer approval, then discover that the approval queue has become the new bottleneck. Routine replies wait beside sensitive billing or account decisions, so response time begins to depend on reviewer capacity rather than the agent.

Exception-only review reduces that queue by reserving human judgment for low-confidence, high-risk, or restricted work. The boundary should still be tested because a weak threshold can send too much routine work for review or allow uncertain output to pass without it.
